Last week, we held our second Patient Family Advisory Committee (PFAC) meeting. PFAC meetings are one of the many ways our patients and families are connecting to Independence Plus.
Guided by our mission to set the standard for healthcare at home, PFAC was created to further improve our patient experiences, and most important, patient outcomes.
Patient Family Advisory Committee
Our PFAC meetings provide a “voice” to our patients and families. This allows our administration team to hear real-time feedback about our care and services from the people who actually receive it – our patients and families.
PFAC meetings are also a safe place for our patients and families to share their stories and connect with others who have similar experiences. Many of our patients and families tell us this is the most valuable part of being a PFAC member.
A variety of topics are discussed at PFAC meetings, including:
- Communication and collaboration with administrative staff
- Methods to provide further support and build stronger relationships
- Personal preferences and values
- Medically fragile, technology-dependent patient care and services
Membership
All current Independence Plus patients and families are encouraged to participate in our bi-annual PFAC meetings.
Our meetings typically include up to 20 patients and family members who represent our diverse patient population and administrative team.
Confidentiality
Members are required to sign a confidentiality agreement before the beginning of every PFAC meeting to protect the confidentiality of our patients and families.
We strongly encourage all current patients and families to participate in our next meeting on Thursday, May 1 from Noon to 2 p.m. We look forward to seeing you there!
We also host a quarterly networking group for nurses called IPI CONNECT. Our next meeting is Wednesday, January 15 from 6:30 to 8:30 p.m.
