
CHAP surveys are “unannounced,” and last month, CHAP arrived to conduct our re-accreditation survey. We welcomed CHAP surveyors into our facilities and patient homes, and guess what they found? ZERO deficiencies in all three service lines! CHAP was particularly impressed with our low re-hospitalization rate (especially given the medical complexity of our patient population), our training and education programs, and the percentage of hours that are successfully staffed in our Home Nursing service line on a continuous basis.
This success was made possible by our patients and their families, referral sources, physicians, vendors, and most importantly, our employees who are committed to providing excellent healthcare services! We are grateful to all of you.
